Position Description
Under general supervision by the ED/Inpatient Social Work Director, Transitional Support Team Clinical Social Worker (CSW) provides clinical social work services consisting of discharge planning, continuing care services, crisis intervention, mandated reporting, information and referral, and coordination of health and mental health services with community providers. The primary function of this role is to provide clinical assessments of patients with substance use disorder, assess for change readiness, and introduce them to treatment settings, including withdrawal management, outpatient care, inpatient care, and programs that provide medications for addiction treatment (MAT). After assessment, CSW will provide appropriate clinical referrals and assist with appointments and other linkages. The primary location for this position will be Hillcrest, but there will be a requirement to float to our East Campus location as needed. Depending on experience of the selected candidate, this position will be hired at the Clinical Social Worker II or III level. While not required, a cover letter is highly recommended when applying to this position.
Qualifications
A Master's degree from an accredited school of social work (MSW). Minimum two (2) years of post-MSW Social Work experience is required to be hired at level II. Must possess the knowledge and abilities essential to the successful performance of the duties assigned to the position. Excellent interpersonal and communication skills. Must be able to work within a team environment.
