The team seeks a highly dynamic nurse who, under the direction of the medical director, will coordinate care for rapidly growing adult congenital heart disease population. Key duties include medication refills, results review/advice, patient assessment and diagnosis planning; triaging patient calls; procedure coordination; case management; medication prior authorizations; insurance justifications; and patient education. The individual must be able to communicate with patients and their family members in a developmentally appropriate and compassionate manner. The nurse coordinator will work alongside the senior adult congenital nurse coordinator, nurse practitioner, and admin staff. Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care While not required, a cover letter is highly recommended when applying to this position.

Qualifications

Must be a graduate from an accredited school of Nursing. RN License issued by the State of California. BLS/CPR and ACLS certification at time of hire with commitment to get ART within six (6) months of hire date. Must have a minimum of five (5) years of active RN nursing experience. Must have a minimum of two (2) years of recent (within the last five years) outpatient clinic experience. Must have a minimum of two (2) years recent (within the last five years) cardiology experience. Must have a minimum of one (1) year experience with triage and in-basket management. Proven/Demonstrated knowledge of Congenital Heart Disease. Must be able to navigating multiple competing priorities. Excellent analytical and critical thinking skills, with the ability to quickly analyze problems, determine appropriate level of intervention, and develop and apply effective solutions.
