Position Description
UC San Diego Health is seeking an outstanding pharmacy technician to join our Transitions of Care team, playing a vital role in ensuring accurate medication histories for patients admitted across our hospitals. Under general supervision, the Pharmacy Technician is responsible for obtaining, documenting, and maintaining accurate best possible medication histories (BPMH) for patients according to standards established by the Department of Pharmacy. This role supports transitions of care initiatives by ensuring high quality medication histories are collected upon admission to promote medication safety and continuity of care. The Pharmacy Technician will work collaboratively with pharmacists, providers, nursing staff, and interdisciplinary teams to identify and resolve discrepancies in patient medication histories. Responsibilities include interviewing patients and caregivers, contacting outpatient pharmacies and providers, reviewing external records, and documenting findings in the electronic health record. This position plays a critical role in reducing medication errors and supporting safe patient transitions. The ideal candidate is a self starter who is accountable, detail oriented, and able to work with minimal supervision in a fast paced healthcare environment. KEY RESPONSIBILITIES: Obtain accurate BPMH through patient and caregiver interviews Review and reconcile medication information from multiple sources (patient reports, outpatient pharmacies, provider offices, prescription databases, prior records) Identify, document, and communicate medication discrepancies to pharmacists and care teams Accurately document medication histories and updates in the electronic health record Support transitions of care workflows for admissions and discharges Collaborate with pharmacists and interdisciplinary teams to improve medication safety Assist pharmacists with medication access and transition related tasks as assigned Adhere to departmental policies, procedures, and regulatory requirements Maintain patient confidentiality and comply with HIPAA standards Participate in quality improvement initiatives related to medication reconciliation
Qualifications
Certified Pharmacy Technician (CPhT). Pharmacy Technician License issued by the California State Board of Pharmacy. Minimum three (3) years of relevant experience required. Experience in project management/system responsibility (e.g. Pyxis, narcotic control, QI, or pharmacy buyer/biller/analyst); OR is a Pharmacy School graduate; OR expanded responsibilities over several areas OR possess a Pharmacy Intern License.
