We are seeking a versatile and caring Primary Care or Geriatrics Physician to join our team, delivering in-home medical visits, skilled nursing facility visits, and telemedicine services to our senior patients. As a Primary Care/Geriatrics Physician with ‘UCSD at Home’–Population Health Services, you will provide high-quality, personalized medical care to senior patients through in-home visits, skilled nursing facility visits, and telemedicine.
Qualifications
Medical degree (MD or DO) from an accredited institution. Active and unrestricted medical license in California. Board certification in Family Medicine, Internal Medicine, or Geriatrics is preferred. A current valid Driver's License in good standing and the ability to travel to patient's homes and Skilled Nursing Facilities throughout San Diego County. Proven experience in primary care, home visits, and skilled nursing home care, particularly in geriatric medicine. Strong interpersonal and communication skills, with the ability to build rapport with senior patients and their families. Compassion, empathy, and cultural sensitivity when working with diverse senior populations. Strong organizational and time management abilities. Familiarity with telemedicine platforms and technology. A commitment to providing patient-centered, in-home and telemedicine-based medical care.
