Position Description

Under general supervision, and in addition to performing the full range of operational level duties, the Clinical Social Worker is responsible for administering a social service program or relevant work. Provides social work services consisting of discharge planning, continuing care services, and clinical social work services, including crisis intervention, mandated reporting, information and referral, and coordination of health and mental health services with community providers. The social worker is responsible for the education and advocacy of patients, families, and/or significant others. Participates in related department functions, such as community and program education. The Clinical Social Worker is required to work in all acute medical units, including the NICU and burn. Complete assessments in the NICU within two days of admission, complete weekly and discharge progress notes, and has the ability to case find. Other duties as assigned. Depending on experience of the selected candidate, this position will be hired at the Clinical Social Worker I, II or III level.

Qualifications

A Master's degree from an accredited school of social work (MSW). Must possess the knowledge and abilities essential to the successful performance of the duties assigned to the position. Excellent interpersonal and communication skills. Must be able to work within a team environment.
